-Crusader Strike does only apply on a rest.
It should check and re-apply also on item switching.
(Example wis13+3 from ring: If I switch my wisdom ring for a lore ring, then put on the wisdom ring again - I'm stuck with only +1 divine bonus)

It does not reapply if a weapon is switched.
Casting it again does not provide the +5EB on the newly equipped weapon. (Dismissing the spell does not help also, only a rest does.)

Sadly, still bugged.
Does a normal Paladin's Holy sword have the same issue? If not, shouldn't it be easy to fix?

Both spell and crusader Holy Sword have this issue, as do all other weapon effect spells. These spells all set the time they were applied and check the time to determine whether a new one can be cast. This is presumably to prevent you from applying the effect to multiple weapons simultaneously.
It is not an easy fix to make, I'm afraid.
A fix for the first issue will be in the next patch, however.
