my opinion is that shouldn't be "in-game" as in a physical presence etc. its fine if you say you have child off in some other settlement, bg, waterdeep etc. The moment when they a player starts saying "is carrying a small child" is when it should be shut down and against server rules. That in itself will lead to pg-13 violations.

Well, having children would be basically a good reason to retire a character. Or do you really see someone slaughtering goblins with a frying, while simultaneously breast feeding fraternal twins?

Pregnancy as such is fine I think. Many forgotten realms novels mention it, some even go as far as writing that some bouts of fighting contributed to a miss-carriage. And Wizards of the Coast also uses a PG-13 policy just like this server. So if its in their products then it should be okay for us too.
Ofcourse: dealing with kids and violence is a whole different topic from pregnancy. I've yet to even see player who played a pregnant character for an entire full term.
And yes, as mentioned the DM's have previously stated that playing under age characters is not permitted because it will quickly escalate to violation the PG-13 policy. So if you have a child you'd have to abstract it and make sure the staff are going to be okay with the things you roleplay in relation to it.
Having kids die isn't forbidden by a PG-13 rating though, just as miscarriages aren't. But you can't be explicit on the circumstances or violence. But that's obvious to everyone already.
Giving birth would be a good reason to retire too, as Simian writes. But not all adventurers necessarily do that. Some decide they'd be bad parents. Which might lead an orphanage. And no, orphanages and orphans aren't a violation of PG-13 rules either. See the Disney movie Bambi, about the orphan deer. (and the origin of the alleged "bambi" complex that most western adults suffer from)

Also, what if the 'guy' doesn't want to have a kid? I've seen it on another server where some chick just pulled a Jerry Springer and said "yeah remember that one night of RP we had where we blackscreened it? Yeah I'm pregnant."
Skip pregnancy all together, nobody wants to RP with a preggo because it's just awkward.

Ah, I had forgotten how narrow minded you can be! But since you brought it up, have you really not seen the movie Junior? Because that movie alone is a reason enough to foster the idea of male pregnancy. Not to mention that straight guys do find their ladies, eventually. Well thanks Simian, I took your advice and went to my female roommates and asked them for advice on roleplaying a pregnant woman. I tried to explain that I didn't want to be as narrow minded like the rest of the world and I want to know what my future spouse is going through, so I decided to roleplay a pregnant woman in Dungeons and Dragons. Well, I think we need to implement some penalties as a pregnant PC progresses in their term. How about a negative DEX every 3 months?
