Seeking clarification: if AT reduced to ONLY 3 attacks, and for a TWF build, what is the sequence? Is it main hand / off-hand / main hand? Or, does the offhand weapon get completely ignored? And, where does the offhand sit in the 1/1/1 attack sequence? Thanks.

The sequence followed is the first three attacks are kept. So assuming a PTWF greater flurry monk the wiki indicates the attack sequence looks like:
NWN2Wiki wrote:For example if a character with BAB 25 were using Perfect Two-Weapon Fighting and Greater Flurry of Blows with two kamas, he or she would receive a total of 15 attacks (5 on each hand from 25 BAB, and 5 more from Greater Flurry -- see bug notes). The attack progression could look something like this:

+25/+25/+25/+20/+15/+10/+5/+27,+25/+20/+15/+10/+5/+0/-5
Thus you would get all 3 of your highest attacks and the benefits of losing 12 APR. We are currently discussing both AT and SF in QC due to the stacking of attacks, but personally I don't see major issues to address. Just note that we are discussing to consider the stacking of PTWF with massive attacks per round to consider if it is an issue.

But you didn't answer my question!!

And what if you don't use Greater Flurry?

I thought I did, but I'll elaborate.

NWN2 has a hard coded attack schedule which has all main hand attacks go, then all off hand attacks (except for greater flurry with the bugged extra attack). Thus when using AT (or Sembian Fencing for that matter) you will only get the main hand attacks. In the example above the first three attacks are your 2 Greater Flurry attacks and your standard first attack (which is why they are all at the highest AB) which would all come from the main hand.

If you were not a monk and had a warrior type with 12 APR and 30 BAB it might look something like:

30/25/20/15/10/5/30/25/20/15/10/5

In this case your attacks with AT is 30/25/20, all with the main hand. You would have one attack per 2 second flurry in the round.

Is this sufficient to explain?

Yep! That's all I needed.

Deduction: AT is NOT good for a TWF.
