How do you make a link and rename it?

When you copy/paste a link, how do you rename it and still have it take you to the right place?

To break it down, [url]<the link url>[/url] is the usual code, but when you add = into the brackets like this: [url=] it changes behaviour, becoming [url=<the url link>]<text>[/url]
