I'm posting this as it's own thread because it seems to be unique to my character. When haste is cast by me it grants the AC and ab but not the extra attack. It doesn't grant an extra attack on myself, my minions or other players that I have tested. Haste seems to be working for everyone else. Proof of the bug:

I learned haste through a scroll instead of picking it at levelup. I've unlearned the spell and learned it again from scrolls from both Baldur's Gate and Candlekeep to the same results. The scrolls themselves when cast do the same. When my character uses an item with haste (5) 1 Use/Day it also does not give me the extra attack.
I would already have rcr'd my character for this bug but I've sunk a lot of gold into my spellbook because I had to choose the new spells at levelup.

The bonus attack from haste is not shown in your character sheet. Have you actually tried it in combat?

Yes that's when I first noticed it.
My immediate plans to maybe solve this are to unlearn haste and pick it at my next levelup. The only thing that I can imagine this as being aside from some bizarre untestable bug is the fact that I learned haste from a scroll. I don't have any other chars on my own account that have access to level 3 spells to verify that this isn't somehow tied to my files or something.
EDIT: Update to this, I picked it at levelup and it does now give me and my companions a third attack but is still not reflected in the character sheet. A bit annoying in parties maybe but I don't have the 20k to spend again on spells so I'll live with it. Move the thread to solved if necessary.

Haste's bonus attack never appears on your character sheet, only in the combat log (final attack of the final 'flurry', always at full BAB).

Not a bug.
Extra attacks from any source besides monk's flurry will never appear on the character sheet. It's just the way things are coded.
If anything, the anomaly is extra attacks from flurry actually SHOWING themselves on the sheet, as they do not for Haste, Flee the Scene, Rapid Shot, etc.
Flurry and Greater Flurry both also give 1 more attack than they're supposed to, but I believe the mode is hard-coded like most (all?) of them, so, there's not much that can be done to change it except completely externalizing them.
