What AC to aim for?
What required AC would you need to achieve to avoid major problems?
And what items could I use to max AC?

Aim for an AC that is 21 points more than the AB of the creatures you're fighting.
But seriously, depends on your PC. Do you have other ways to avoid or reduce damage? If not (e.g. you're a straight up fighter with no avoidance tricks) then you'll want anywhere between 45 and 50. If your damage is at the high end then you can get away with slightly less AC (45ish) because you'll kill quicker and the enemies will land less hits. If you're more of a grinder style, then get your AC higher because it'll take longer to kill and the AI will roll a lot of 20's!

For example I have a duel axe wielding dwarf with pretty high AB but I'm a little concerned about later levels with his AC.
His build will ultimately be (at this stage) AC 26 naked with a +3 dex item, he wears light armor, what else can increase his AC? and will his base AC be enough?

Around 50 is advisable by the endgame. Any less and you will need to be careful in pve. Events generally are geared to favored souls so less than 60 Ac will make you an easy kill for the mobs.
A duel wielding character without hips typically tops out around 40-45. Be very careful and bring plenty of healing. Mages can offer stoneskin and these are uber helpful.

If you'll have good damage (300+/round) and regen/vamp regen, 42-43 ac may be enough up to and through frost keep.

40-45+ is decent for most mobs and will give you some protection against epic mobs and or bosses. 50-60+ will make you almost invincible except for a few specific and rare mobs... such as the magma golems under Durlag's, those things will hit you consistently even if you have 65+... and do ungodly damage as well.

What combination of items can give you + 4 dodge, +4 deflection and +4 natural AC?

Dodge is only on boots, natural armor only on amulets, deflection can be just about on any item, now items with such bonuses can be found at that Avernus shop but otherwise there is always the random loot generator.
