Why is it so much lower in CR than Titanfist Mine for instance?

I would put it closer to the adjacent area.

Yes, all camp areas were designed for CR2 lvling. That area is actually on my list for deletion/recycling since Dwarves never use it.

If you add the conversation going down to West of Grottoes from inside Kraak Helzak to the outside of it - the caravan going to BG city (similar to Darkhold) - it might help making this caravan more used.

As it happens my dwarf still uses it, although it is a relative long walk to the dwarf, who seems unconspicous and many will just disregard as another guard.

All 4 of the 'of Grottos' areas are a bit small - would be great if they could be connected, as all 4 are low level zones. (South, West, North West, and North of Grotto).

I was just there (South of Grotto) today with my lv2 char. and there is really not much to do.

An yeah, if you get to West of Grotto as a lowlevel dwarf, you can't get anywhere else, as the only deeper way is a huge increase in difficulty.
