Is it only in certain areas? Only from certain Bosses? Just curious. Never owned any before.
Thanks!

It can drop pretty much anywhere at varying chances. I once found some Epic Fighter Boots (4 AC, 10 HP, 4 intimidate) in a chest inside a CR15 area. Your chance of finding something really good goes up with the difficulty of the creature you're fighting though, as the RNG loot generation is based on the CR of the monster you kill (higher is better).

Hands down the bestest best place for loot drops is the Vault of Death in the Fields of the Dead. I find 90% of my really good stuff there.
Best 2 hour loot run for epic items? VOD > Ice Giants > Amn Lizards > Necro Cave in Amn Peaks.
Run that daily and you will find an epic item within the first 3 months....give or take a year.......

I've been with this server for 10 years and I've yet to get an epic drop.

Yes, drops are very rare for epic items. Just keep in mind when looking for it.

i have probably gotten a half dozen in the last year. more than half of those in non epic areas. must also understand that some items can be just as good and better than epic even though it is not with a typical +4 attribute with extras.

Where does epic gear drop? Everywhere and nowhere ...

Also, no-one is to shake me.
Seriously though, it's a numbers game. Open up a heap of chests. That gets boring after about 5 minutes though, so by far and away the easiest and quickest way to amass epic gear is to trade. I heard Karond say it years ago that he didn't even loot-run anymore and I thought "Yeah, right. Not likely." But he was right, I never hard core loot run - haven't for years. I open chests if they're there or if I'm adventuring in a group. But sometimes I just don't bother because I know the chances are slim and the next epic trade is just around the corner anyway. When you find an epic item don't sell or trade it straight away. Try to uptrade. Try to get slightly more than it's worth and then just rinse/repeat with your new item(s). Golden rule: make sure all trading parties walk away happy.
Edit: Also, I know there's always a "Keeping up with ..." factor, but consider the fact that given enough gold everyone can have an epically outfitted pc now. This is a huge boon that never existed before. Epic stores offer such good items now. High appraise merchant pc's make them more affordable. It really isn't a matter of 'if' but 'when'.

oh.. and try not to have 15 characters that you are outfitting. gear and gold pile runs thin when you have a bunch of hands in it.
