Excruciarch with AB of 48, with bone devil that has AB of 38 next... Right off the bat at area transition? The Excruciarch did about 40~ damage per hit... There was no time to even run away.

Honestly, wtf? This used to be CR 10-15 area. What has happened? There should be a warning sign just outside.

Or this the work of the recent problem child?

Nope, by design. People have been RPing about it, actually. "Devils have overrun Dragonspear Castle," a group of characters has set up a watch just north of Soubar, etc. etc.

How about an actual warning sign... That you are going to enter some CR 45~ area...

I mean, if a CR 15 area called Fields of the Dead had such warning sign... maybe Dragonspear should too.
