It never happened before. When I try to summon a familiar nothing happens, I am just wasting the use of this skill.

This happened to me too. Use the Bind Familiar feat instead. You'll be happier with the results.

I was most likely killed at some point, not sure if you would be able to summon it after a reset.

Bind Familiar seems to be the replacement.

Summon familiar doesn't work
