Double Constitution Items

I noticed having two Constitution Items deals damage to you when logging in or coming out of a polymorph transformation like bear warrior rage and druid shapeshift.
Anyone know why? Is there a fox for this?

Yeah it's to prevent an exploit, the fix is to not have two items with bonus constitution equipped. At worst ask a DM to remove the CON bonus on one of those.

But asside from removing the second con item, it's that or take the damage?

Without this fix anybody can get infinite hp so yes.

I once, with a DM, gained 5000HP+ being polymorphed as a frost giant.
