Skill Points.

Do skill points stack on equipment? So if I had a +4 perform on a helm and a cloak would it be +8 on my performance skill?

That is correct, to a maximum cap of +50. This cap includes bonuses gained from spells and abilities (eg. Heroism or Inspire Competence) but not feats (eg. Artist & Skill Focus Perform do not count to the +50 cap)
