Constitution adjustment on Bear Warrior with CON Belt

My bear warrior gets hit with the constitution adjustment damage twice in a row when shifting back from Bear Form, started happening with a Belt of Growth. I'm assuming that, unlike CON buff on a helmet, the belt is unequipped during Bear Form , so shifting back to human hits my character with the adjustment from Bear Form then again from the +3 CON on the belt? 84HP drop after a rage is brutal X)
