Frost Giants' Can't Count

The screenshot says it all, but to recap.

A character with a 50AC is attacked by a Frost Giant Warrior. The FGW's attack rolls are [17 + 31=48] and [17 + 26=43] against a 50AC. Both attacks hit. How?

Does the FGW have now sneak attack?

Does your PC have Uncanny Dodge? Maybe it got Flat-footed.

No Uncanny Dodge. His reflex save is 30. Does that count?

Flat-footed is being especially vulnerable to attacks at the beginning of a battle. Characters are flat-footed until their first turns in the initiative cycle or after the first flurry of attacks by a single opponent against them, whichever comes first. A flat-footed creature loses its Dexterity, Tumble and Dodge bonuses to Armor Class (as well as penalties) and cannot make attacks of opportunity.
I can't see earlier in the Combat Log based off your pic, but perhaps the FG one initiative here—it looks like maybe since you tried to Charge the FG with your PC, meaning combat was just being initiated—and by winning initiative, and likely have 2 attacks in the first flurry, the FG attacked your PC as Flat Footed, thus your PC lost all Dodge-based AC.

What is your PC's AC without counting Dodge types?

Dexterity bonus is +3
Tumble is +0
Dodge is +4 (boots)

43 AC flatfooted

The FG rolled a 48 and then a 43. That would be 2 hits on a 43 AC flat footed!

Exactly. What is not clear... is how my character, any character becomes disadvantaged (flat-footed) in single combat? It happened all the time to my Wisdom-based monk, but that was typically against mobs.

The exact rules can be found on NWN2 wiki, but in this case, it was most likely initiative. You can see that you only roll initiative after the first attack - this implies the mob started the engagement, therefore you were flat-footed for its first round or flurry of attacks.
