In the Lower City, The Steeps. At the Counting House, we've got some hovercrate action going on. The terrain at the door might need some adjusting too, unless the poor folk inside are meant to be stuck there.
The cute halfling homes in the district sadly lack doors.

In the Seatower District, the Hissing Stones Bathhouse has a rather short looking door. The building is also inaccessible. Intended?
Some other terrain-challenged doors were found in the top-mid of the district.
The entrance to the Manorborn district from the Seatower district has a wayward flag clipping about in the ground.

Manorborn district contains a bench in the Sashenstar Estate where tieflings from all around the world can congregate to test their hornstrength.
In the top right of the district, near the sewer entrance and House Vale, ghosts have displaced the doors.
House Vale's door to their shrine of Siamorphe can be opened, but not entered. I was able to tumblejump up through the building's main entrance to land on top of it.
Halls of Inner Light: Correct if I'm wrong, but should it not be inaccessible in it's current IC state? It also contains NPCs who died in ToT.
There's a random smoke effect hanging around in the sky, presumably tied to the lighting over this particular spot in the district.
The fence outside the Tymoran temple has some weird elevation going on with it.

In the Temple's District, the Temple of Torm has some weirdness going on the southern side of the building. They also have the world's tiniest archery range. There'd be more room for that stuff around the back.
The structure around the ducal palace has run into some issues. East of the palace there's a missing door and some terrain issues.
In the top mid of the district, close to the City Watch Citadel, there's another home without a door.
More door strangeness with one of the towers.

In The Wide, I've run into more of the oddly short doors, as well as some visual funkiness with the steps opposite of the shrine to Lathander. It's very noticeable when moving the camera.
Inside the Shrine, the corners of the rooms seem to have some issues. The walls themselves are rather camera unfriendly.
The big gate under the new Thayan enclave has a door several sizes too small.
This building has a false transition stuck into it, listed as a sewer entrance.
The transition from the Wide into the Heapside district seems to have chains going from the portcullis passage into the ground. I assume the asset is from a drawbridge, hidden under the terrain.
