By which means it is normally possible to acquire permanent wound?
I've asked this question before and I've been told that it is:
1. Messing with lower planes.
2. Cursed weapons.
3. Curses.
Anything else? I have impression that wizard can inflict wound like that. The question is - how?
P.S. It is backstory matter.

Technically, Wizards could transmute your character in such a way that the wound doesn't count as a wound for healing spells. A permanent transmutation, if you will, like Mass Fowl or Flesh to Stone. Healable with Wish, Miracle or a counter-spell.
Like, instead of removing your eye... they cast a spell to transform you into a form that looks exactly like you but doesn't have an eye.
A counter spell would essentially turn you into a form that looks exactly like you but has an eye.
Developing a counter spell would be dangerous in case the target form has some kind of mistake in it. I really love accidental belts of masculinity/femininity

Wishes are obviously difficult. Miracles? Gotta convince the god to do it - quest/conversion.
And honestly, if you make it that the wizard who did it is strong enough to do a permanent transmutation (meaning it looks the same with True Seeing and cannot be dispelled/removed with dead/anti magic zones and stays that way after you die), they will have a decent caster level. Technically, as long as nobody tries Anti-magic zones (like throwing your PC at a beholder), it's a possibility PCs, unless collaborating, won't be able to dispell a normal spell either. However, True Seeing would show your PC with a functioning eye. So I guess a proper Flesh to Stone but Eye to wound would work better.
It's healable, but not with healing spells and it would take careful research to counter or taking a risk with Wish or getting in the good graces of the god asked for a miracle - in short, you don't run risk of random guy running up and casting Regenerate.
