Are there any guidelines for running masked character on surface (meaning that character refuses to take off the mask in almost all circumstances)?
The main issue is that acting this way will lock character out of many vendors (the main problem is equipment that can only be purchased in BG), some quests/gold sources, and it will be somewhat difficult to pass by baldur's gate (because you'll get yelled at by FF guard and imprisoned).
I'm aware of most of round-about ways to travel maps and at least one city where it is possible to exist with mask on, but I'd be interested to know if I might be missing some information.
IIRC there were at least two well known permanently-masked PCs - Z'athan and Allison.

The short answer is that the difficulties in wearing a mask in the places you mentioned are indeed intended IC difficulties. I do not think you are missing anything if you know the routes round and the fact that in two settlements this won't be a problem (Soubar and Roaringshore). Note, its intended to add a low level vendor to Soubar.
Long established characters can earn a special dispensation in one or more settlements for whatever reason, eg being hideously deformed. Even then it might be required to RP showing one's face at the gate.

One more question, though: Is harbor area of BG as secure as the rest of the city? IG, the FF headquarters are in the Harbour, and there is also a giant sea fortess (off map!) that defends the Harbour in lore.
That said, yes its true its easier to sneak in via the harbour. But you would need a DM to supervise interactions with Flaming Fist guards and civilians if you attempted it.
