Earth Elemental Guardian drops wrong keys
In the Underdark, East of Mistlake the boss drops two Forest of Wyrms keys which don't open the chest in his lair. Probably just a copy from the surface in the Forest of Wyrms.

You are correct, the boss is a copy and paste from the surface. The Golems are teleporting in from somewhere, but you are the 2nd player to bring this up. I'll add it to the list if you feel this is game-breaking.
