I'm thinking of a character inspired by Darkest Dungeon.
For those who have not played it the character is a leper with a big giant sword that he can one hand swing. So great sword and melee front liner.
And of course rp he would be suffering an advanced form of lepresey. Not sure how that would work on a realm of magic but in the old days leprosey was pretty much a death sentence and the biggest social outcast maker in the world.

My first thought: it should be something with the spell Abyssal Might at its disposal - for cosmetic reasons.

With all the magic healing going around... always tough to play "diseases" "uncurable" stuff...

Well, not all disease can be cured by magic. Magical disease could be tough.
Lycanthrophy is a disease in Forgotten realms and it can't, as far as I know, be cured easily my magic.
Some disease, such as Halhruaan Consumption,made magical healing spells less effective.
And we do have gods of disease. I bet they would love mucking around with nature to make disease more stronger to magic.
I think though, magic cures would be more effective if they were used to treat the sickness at the early stages. Like how sickness in real life is easier to treat if caught early, but untreated they could become harder to cure.
Looking through the Forgotten Realms wiki I found a page of diseases that ravaged areas in Faerun. So we know that even with magic, disease finds a way to cause havoc.
And it could be a magical form of leprosy, like maybe a curse by Talona or something. A curse by a god would definitely not be easily cured by some cleric on the street but would need say a ritual or a special item, like say water from glade of Eldath or some other thing.

So blackguard. Though, if sticking to the theme of the leper he would be a holy warrior.
I was thinking more along his stat line before classes.
Like maybe a low dex score and for sure a low charisma score, leprosy is uh....not a pretty disease. So like 8 in those two scores. Maybe also 8-10 constitution if its an advanced form.
Would need something that offers damage reduction to make up for the weak hp. Though Heavy armor will help with AC.
So maybe barbarian/weapon master/divine champion? Can divine champion even be non lawful?
So blackguard. Though, if sticking to the theme of the leper he would be a holy warrior.

Warrior of darkness seems a good call?
Can add a cool glow to your weapon too + dmg reduce and generally just a great class.
Though if you want more dmg reduce need to think 21 con.
