The tails were developed for the NWN2 campaigns, where a companion NPC (http://nwn2.wikia.com/wiki/Neeshka) has one. We didn't create this as custom content, we just gave tiefling players the ability to toggle tails on or off based on what sort of tiefling they want to roleplay.
The lore of the class suggests there are many different ways one's fiendish heritage might be portrayed. Some of the examples in Races of Faerun are pretty subtle, and some are less subtle.

Fair enough, I do remember Neeshka haha. I can definitely say I don't like what they did with their appearance in 4th and 5th ED. I have just never been much for giving players too much leeway and such on their PCs for appearance(such as wings or letting them be vampires/angels and all that) or story. Then I think they just make the most outrageous backstories haha
