Have you never seen an actual road sign before? If something like 'BEWARE DANGER' is written on it, it will overlap the actual text on the sign. It will not be a note stuck at the back of the sign.

If you can't figure out theres more text on the sign, when your cursor hovers over said sign. I don't know how to help you.
You're repeatedly asking me if I know how signs work. Yes, I have eyes and I'm not stupid. Would you prefer it if the ingame sign simply had a skull and crossbones over it with no additional text or context, no floating text over it? Would that be any easier for you to see and comprehend? To me, that would be much more difficult to see. That is the whole point of the game's design in reference to the click point examines, so you can get a description of what otherwise cannot be represented or observed graphically. But that is how signs actually work. Pictures and letters on a typically flat surface. No hovering text over them or view-able by clicking on them.

The fact of the matter is that the information is on the sign.
Does it appear in its entirety when you hover over the placeable? No. But neither does the full text for plenty of signs IG, or for valuable information that requires examining for all sorts of other IG landmarks that would otherwise be “immediately visible.”
The sign’s added info also says it’s been scratched into the front of the existing sign, appending what’s already there. Unwieldy tooltip aside, it seems like there are plenty of ways to handle this perfectly IC.
For example, perhaps in haste to reach the keep and lay low some orcs for what’s expected to be easy treasure, a character in a hurry thinking everything is as it was did not take the time to read the message carved into what seemed to be an innocuous sign without warning—which does, in fact, contain a warning.
On an OOC level, the same character’s player, once stumbling upon the area and realizing the difference, would surely now realize such and thus not have dire need for a tooltip to make it clear in the future.
The same player might also go about learning more of the situation IC, perhaps leading to role play in order to warn other characters of the dangers of the region at present, as numerous PCs have been dedicated to doing for going on two weeks since the changes in the module were implemented.
