You can't end it in the Nexus and require a DM to teleport you down below to recast the spell to end the effect, It persists through server resets, which is strange.
Again, you can't remove the disguise effect with the disguise tool. You have to recast the spell to end the effect. For now, It's permanent, and Mystra won't let you down while disguised.

I use the disguise spell daily and have never had this problem.
A few times I had a disguise 'stick' to my original character name, but that was simple to circumvent. Use mundane disguise to rename character to [keyboard mash]. Then immediately remove it. Talk to Mystra. Good as gold!

I tried this earlier with DM Imposter, the disguise tool will not remove the disguise spell from my character, only a recast of the spell removes it. You can't cast in the nexus. So Mystra would not free me from the Nexus. He had to teleport me to escape the Nexus.
It does in fact persist through server reset. At least for me. I would try it again, but the Nexus prison sucks.
In fact, I'm sure the tool isn't removing any disguises anymore, only applying new disguises. That's why people are probably reporting stretched out character models. I can recreate this in game if you want, but I'd rather not.
