Probably you had a char. with violation before and you RCRed her into the same name without waiting for a reset.
This doesn't clear all your char. data - so the violation would still be attached to the new char.
possible solutions:
RCR her into (a slightly) another name
or RCR (store xp) - wait for a reset - make a char with the same name and reclaim xp
