I always found it weird that bonuses on various items would stack. Like, +3 intimidate on a ring and on a necklace shouldn't give +6.

Why wouldn't it? They always stack.
Abilities do not stack, skills stack.

gear and spell bonuses (heroism, etc ) count towards the +50 cap yes
you basically have
stat bonus
skill ranks
feats
gear/spell bonus
so say for H/MS on a sneak with a 30 dex that caps out hide bonus from gear and spells (impossible to cap MS) and has max ranks, you would end up with
10(stat) + 33(ranks) + 6(skill focus +3, stealthy +2, wildchild +1) + 50(gear/spell) = 99
mine hits 97 because i dont have stealthy

So all skills are cap at 50 (gear/spell)?

Bonuses from familiar don't count towards the 50 cap to my knowledge.

I think the cap is 100... but most people only reach around 90 im told.

i think someone said the hard cap was like 120 before the numbering resets and bugs out but i cant remember, you would be hard pressed to exceed 100 anyway
and familiar bonus is applied as a feat

Looking around in nwnx dev forum I stumbled on the info that in
NWN1 skills go up to 127 (which makes some sense computer wise) and then roll over, so if you have 128 the game considers you have 0 etc.
If the same holds true for NWN2 then with the 50 bonus cap and 33 maximum ranks you're still quite far from reaching 127.:
Assuming you roll 20, get the 50 bonus and have the 33 ranks you would be at 103 and still need 24 points in vanilla feats and relevant ability modifier concerning that skill to rollover.
I think druids and spot skill with aasimar and all the spot feats is the highest you can get : 42 WIS for a 16 wis modifier, +2 spot from racial feat, +3 skill focus, +2 blooded background feat, +2 alertness => 25, so a druid could technically reach 108 base spot, roll 20 on a detection and be at 128 = blind

you forgot the farmer feat which is another 1 or 2 but yea, would only be possible with spot since it has the highest bonus from feats

In the game which nwn2 is supposedly based on, it doesn't, no. And I don't think it would 'break' things to put it like it should be, it would just do that people wouldn't carry gear for X or for Y and keep changing clothes all the time on the tradeway, which is pretty weird.

It would break because of the design of the server. There are areas designed around specific stealth scores based on having stacking gear. If stealth gear stacks than listen/spot gear must also stack.
Any build based on feint already struggles to actually make use of the feat due to spot scores of some mobs, that would have to be adjusted too. It would take a LOT of development to re-engineer the entire server around gear that doesn't stack skill points.

Yet, but in my opinion, it would make it less weird. I don't know if people like it or not to be carrying 'Stealth' gear, 'spot' gear, 'perform' gear and so on in their bags, and see rolls in the 90. I just know that personally, I always found it strange.
