Imo, this whole (core) issue is really a none issue. RP outs are just a way to enforce the fact that 99% of PvP (the other 1% consisting of DM assassinations, certain classes, and rulebreakers) will require RP first. You could do something like:
Druid sees Necromancer prancing around the Wytch's Cave with a vampire in toe.
*The druid shapes into a dragon and lets out a menacing roar, baring its fangs in a hostile gesture. The "dragon" stretches its wings back, in clear preparation to charge the necromancer.*
Druid sets hostile.
Now that the RP out is given, the necromancer has two options. They can then run (take the RP out thusly not consenting to PvP) or stay (and lose the RP out thereby consenting to PvP).
The RP out rule ensures that the druid has to RP the move to attack, to give the necromancer time to react. Else-wise, the druid could attack the necromancer whilst they are under lag, afk, etc. Which, would not be particularly fair or fun. It doesn't make it so you have to seize up your RP, however. Its not a restricting mechanism, it just ensures everyone is on the same page.
Regarding summons, it would be nice if you could set a summon hostile. Not sure if that is doable, though.
