It seems many a character has suffered #20!
Number 26 would certainly be fun for a Chauntean or a Druid.
I might end up culling this list for the ones that wouldn't mechanically impact anyone else or too much of the world and roll apply them to my character. We could start a new club

Currently running a Curse of Strahd campaign for tabletop. Because souls can not escape the demi plane anyone brought back to life suffers from a random madness effect as their mind can't handle that knowledge.

Great list, Israe! I'll have to keep this in mind if one of my characters ever comes back to life through a true resurrection.

I think these work better as curse ideas. Lore-wise, there are no side-effects from a True Resurrection. True Resurrection however is very expensive.
A Raise Dead costs 5 000 gold in diamonds, Resurrection 10 000, True Resurrection 25 000.

If you'd want to RP a side-effect, a temporary hit to CON and thus feeling weakened for some time seems more accurate. I wouldn't propose a permanent effect.

Means you can totally make it a character goal to bring back your childhood friend from 30 years ago through True Resurrection

Personally I would think for a resurrection their is always a chance for your soul to be corrupted. Or something else came back with your soul depending on the length of time your soul was disconnect from your body. Perhaps kind of like a sickness that distorts the way your character perceives their of relationships with others, morality, slight change in behavior, or something along those lines.

It seems a bit counter-intuitive for a True Resurrection spell to come with risk of such side-effects, rather than Raise Dead or the j-average Resurrection, considering the purpose of the spell (and reason for such a high caster level/material component requirement) is to be the best way to bring a creature back to life with no complications.
Anyhoo, for those curious, the D&D splatbook Heroes of Horror (2005) has previously touched on the subject of characters 'coming back wrong' from the dead on p. 79-80 for the purposes of horror-themed campaigns (with their own Resurrection Mishaps table on p. 80).

My merchant has many items that grant wonderful penalties to Con, if you wish to represent this weakness in-game. Usually, they are reserved for prisoners, but I can make exceptions for a price.

you also have to take in to consideration how long they have been dead and/or do you have there body. As well as what kind of condition that body was in when casting. Even the strongest of spells have some kind of price other than monetary value. Some spell age you for example the haste spell.
