Upperdark: South of Grottoes.
The transition between the outpost and the rest of the cavern in the South of Grottoes are gone, so you don't have access to the upperdark from the underdark that way anymore.

Can someone post a minimap screenshot of where the transition should be and in which area ? "Outpost" is unclear for me.
