Bombs not working?

Just attempted to use Grease Bombs and Flash Bombs. Neither seem to have any effect on hostile creatures (no VFX either).

What's your set traps/disable device skills?

I have 50's in the skills after gear and dex mod and can arm them fine, but when they're thrown they trigger no effect and no save and the item is consumed.
