Ogre Chieftain casts spell breach without casting.
Just encountered the Ogre Chieftain in the Troll Claws cave, it or one of its mages casted a Spell Breach on my character, without it appearing in the combat log. All other spells the Chief and Mage casted appeared in the combat log. Only noticed due to the expiring icons appearing above my characters head.

More likely a trap, that ogre mage is a cleric and doesn't have breach.

Pretty sure the Pit Fiend does the same thing in Nashkel Mines. As soon as he spawns the breach (or greater dispel, don't recall which it is tbh) fires and can't be dodged/HiPS'd to defend against it. Pretty sure the trap/trigger for the boss also triggers the spell at the same time. *shrug* game cheats, so do I...just go back through the door and rebuff so as not to get insta-gibbed by the boss. 
Hopefully some day we have dungeons balanced for difficulty that doesn't allow such simple methods of escape, but also doesn't fire spells that are 'traps' but can't be detected, disarmed or avoided. Until then it is a situation where you can only go in understanding the game, learning and then playing around the shortcomings. It'll never be perfect, it's a game after all, but I do think there are things we can do with dungeon design to make them 'fair' while still making them appropriately challenging.

Just tested the Ogre Chieftan again in the Troll Claws cave.
I can confirm that he does cast a Spell Breach as I watched him do the casting animation(possibly greater as it removes Death Ward and Elemental Immunity in the same go). BUT it does not appear in the combat log. All other spells cast by it do appear in the combat log.
Was able to sit back and watch it cast away on a companion and watch the log in detail. And watch the expiry icons appear above my characters companion of those two spells.
