When I went through the door to get to the lower level of the Nashkel mines I spawned behind the door and couldn't open the door, or get out, and relogging didn't help. I don't know if there was someone steathed or invis who "bumped" me by accident or if it was just a random glitch, but maybe put the spawn in point a bit further away from the door and put the devil on the tracks a little further back? And/or make that door openable from the wrong side?

ETA: I was able to fix it by logging into server 2, which put me back at my last location on that server, which may be a bug in itself

That seems to be a very rare bug with teleportation positioning that may be cause by wasding or having some other movement stuff at the moment of jump script working. We will move it further from door in case things repeat.
